Chanda Plan Foundation Receives Grant from Pinnacol Assurance

The Chanda Plan Foundation has received a $15,000 grant from Pinnacol Assurance to help support its new Care Coordination services now available at the Chanda Center for Health.

The Chanda Plan Foundation recently added Care Coordination to the list of complementary services it provides to improve health outcomes and reduce healthcare costs for persons with physical disabilities. By utilizing the Care Coordination service, participants are provided with a point of navigation regarding opportunities available to them internally at the center, as well as a resource for services offered outside of the center, including Medicaid, transportation, home modifications, etc.

Chanda Center for Health to Offer Care Coordination and Behavioral Health Services

The Chanda Plan Foundation announced that the Chanda Center for Health has added Care Coordination and Behavioral Health Services to its list of complementary services designed to improve health outcomes and reduce healthcare costs for persons with physical disabilities. These new services are now available to all participants at the Chanda Center for Health.

Participants can utilize the health center’s Care Coordination as a point of navigation regarding services internally at the facility and for services outside of the facility. The Chanda Center for Health’s Care Coordinator will help participants understand what opportunities are available to them and make connections at the health center, and they will also be on hand to assist with required paperwork necessary to access needed resources, such as Medicaid, transportation, home modifications, etc.

Chanda Plan Foundation Partners with Kaplan Morrell, LLC

The Chanda Plan Foundation has created a partnership with Kaplan Morrell, LLC, a Denver and Greeley-based workers’ compensation and disability law firm, to provide a dedicated Workers’ Compensation and Social Security resource for its clients.

Kaplan Morrell, LLC will provide 24-hour access to Chanda Plan Foundation and Chanda Center for Health participants who may have questions or concerns regarding all workers’ compensation issues. Kaplan Morrell will also give presentations or talks as needed to participants.

DU Alumna Helps Those with Spinal Cord Injuries

“I have the best job ever. Every day I get to see that we are changing people’s lives and not everybody gets a job where that’s what they feel every day.”

...Hinton Leichtle helps hundreds of people each year find the alternative therapies that will help them live with their spinal injuries. She has worked tirelessly with lawmakers to find funding. And earlier this month she opened a new 6,000-square-foot facility in Lakewood to offer the integrated therapies for those in need.

“This is the place where individuals with physical disabilities can come and get all their needs under one roof,” she says. “Everything we do is collaborative versus competitive; it’s all proactive, and its very therapeutic versus medical filling.”

Chanda Plan Foundation Opens New Chanda Center For Health

The Chanda Plan Foundation opens the Chanda Center for Health this month, the country’s first integrated medical center for individuals with physical disabilities. The goal, Chanda Hinton Leichtle says, is to provide patients with the “quality, person-centered, and streamlined health care they deserve.” To wit: The 6,000-square-foot facility, located in Lakewood, will offer primary care, mental health services, and nutrition counseling, plus adaptive yoga and other integrative therapies—all in a space specifically designed to serve a population used to being ignored.
