The Chanda Plan Foundation officially opened a new health center ushering in a completely new and innovative model of health care for people with long-term physical disabilities.



Location-based services are delivered through contracted providers at the Chanda Center for Health. Historically, location-based services focused solely on integrative therapies. After years of providing this program, the Chanda Plan Foundation identified additional needs for this population, including counseling, primary care, and care coordination. On April 10, 2017, the Chanda Plan Foundation opened the Chanda Center for Health. The expanded space creates additional capacity to deliver more services to new participants. The goal of the health center is to improve the quality of life for individuals with long-term physical disabilities by providing population-competent care through a team-based approach that reduces the cost of health care and decreases reoccurring health issues. The Chanda Center for Health offers: 

  • Integrative Therapies: massage, acupuncture, yoga and chiropractic care.
  • Counseling Services: A behavioral health provider who is knowledgeable about physical disabilities offers on-site therapy to address depression, anxiety, stress, pain management, etc.
  • Care Coordination: A Care Coordinator ensures participants are receiving the services they need, both within the health center and beyond. The Care Coordinator meets with all participants to help them navigate complex institutions, enroll in programs, and access resources to enable individuals to regain independence in the community.
  • Community Building: The health center acts as a community center to connect participants in its multi-use space. A Community Tea Bar encourages relationship-building and collaboration among participants between appointments.
  • Training and Education: To improve provider knowledge regarding long-term physical disabilities and the quality of care provided, the Chanda Plan Foundation is developing a curriculum to educate health care professionals.

I struggled for two years to find such a warm and welcoming place where people understand the difficulties of life in a wheelchair. Front Desk Officer

Shortly after being in a motorcycle accident and being paralyzed, Jake discovered and started receiving care from the Chanda Center for Health. Through the help of massage and acupuncture, Jake was able to reduce his pain enough to go back to work. After working for a school as a front desk receptionist, a year later he applied to work at the very same place that helped get him there in the first place.

Jake has a bachelor’s degree in Criminal Justice, and is working towards another in Cyber Security. He is married to wonderful wife who is a Professional Dog Trainer, and they have two dogs of their own. A Staffordshire Bull Terrier named Tank, and a pug named Shorty.

Jake enjoys working at the Front Desk and helping the participants we serve, especially because he can relate to their experience very well.



Hailey StrampEL

Administrative Assistant

Hailey is the Administrative Assistant for the Chanda Center for Health.  She graduated from Michigan State University, Eli Broad College of Business with a Bachelor of Arts in Materials and Logistics Management in 1993.  Following years of bookkeeping and office management experience, she continued her education at Boulder College of Massage Therapy and became Nationally Certified in Therapeutic Massage and Bodywork in 2004.  Hailey has experience working with both non-profit associations and persons with disabilities having volunteered in various capacities with the American Massage Therapy Association as well as being married to a person with a spinal cord injury.  

In her spare time, she enjoys spending time with family (including four-legged members, Mr. Jones and Milo), seeing live music, camping, traveling, and volunteering.  

She believes that some of the best methods for dealing with the symptoms of depression and/or anxiety are receiving massage, receiving acupuncture, connecting with people, and volunteering.  Her philosophy is that every body deserves more healthy touch in their lives.


Gary Stockman

Behavioral Health Provider

Gary is a licensed clinical social worker and graduate of the University of Iowa with a Bachelor of Arts in psychology and a Master degree in Social Work. His forty- year career includes a variety of clinical and managerial positions working in both non-profit and for-profit settings. Clinically, he specializes in the mental health of children, adolescents and adults. He sees people on an individual, couple, family and group basis. His clients have presented with a diverse array of concerns that he has successfully helped them with. 

Gary is married and has two grown children, and three grandchildren. He and his wife enjoy their time with family and friends, being outdoors, visiting Parks, camping, and relaxing on the back deck. Gary also enjoys playing guitar, exercising, and reading.    


Anne Patton

Care Coordinator

As a parent of three grown children, the oldest of whom has autism, is the catalyst for her career in care coordination.  Anne has had numerous roles in the disability community over the past 30 years.  She has been the Treasurer and a board member at Advocacy Denver, the Care Coordination Manager for a start-up non-profit for adults with disabilities, and the Finance and Operations Manager for Family Voices Colorado, a non-profit providing resources to families of children with special health care needs. Additionally, Anne has supported her oldest son in starting his own business, while most recently forming a Medicaid approved home and community based service agency, providing services for her son under a unique model of support crafted to better meet his needs in living the life of his choosing. Integrative therapies have been a relevant and important tool for all members of Anne’s family, and she is excited to bring her expertise and passion to the Chanda Center for Health.


Program Coordinator

Contracted Administrative Assistant: Rachael is a Colorado native. Growing up in Louisville Colorado, she became a Certified Nursing Assistant at the age of 16. She started as a CNA at Boulder Community Hospital, she worked there for almost 6 years between the medical surgery floor and the cardiac/telemetry floor. In addition to working at Boulder Community Hospital, she started providing in home health care for private clients on her own and still provides private home health care for some clients to this day. In 2012, Rachael became a Massage Therapist. She wanted to continue working with individuals with disabilities. In February of 2014, she became a provider of the CO Spinal Cord Injury Waiver.
